Governor appoints KBEMS members

 

FRANKFORT — The following executive order has been released by the Kentucky Governor's office, through Jim Carreer of the Department for Public Health, establishing the members of the Kentucky Board of Emergency Medical Services:

Executive Order 2000-1247
September 19, 2000

By virtue of the authority vested in me pursuant to House Bill 405, as enacted by the 2000 Kentucky General Assembly, I, Paul E. Patton, Governor of the Commonwealth of Kentucky, do hereby appoint the following as members of the Kentucky Board of Emergency Medical Services:

Mary S. Guidugli, 4112 Old State Road #2, Alexandria, Kentucky 41001, representing paramedics, to serve for a term expiring September 19, 2002;

Julie L. Johnson, 3318 Breckenridge Lane, Louisville, Kentucky, 40220, representing emergency medical technician-basic, to server for a term expiring September 19, 2004;

Joseph Lewis Atwell, 1677 State Route 781 South, Fulton, Kentucky 42041, representing emergency medical technician-first responder, to serve for a term expiring September 19, 2001;

Julia E. Martin, M.D., 238 Henry Clay Boulevard, Lexington, Kentucky 40502, representing physicians having a primary practice in the delivery of emergency medical care, to serve for a term expiring September 19, 2003;

Irvin E. Smith, M.D., 145 Brookside Cove, Paducah, Kentucky 42003, representing physicians serving as a medical director of an advanced life support ambulance service, to serve for a term expiring September 19, 2004;

Mary E. Fallat, M.D., 3601 Woodside Road, Louisville, Kentucky 40222-5962, representing physicians routinely involved in the emergency care of ill and injured children, to serve for a term expiring September 19, 2002;

David E. Bentley, M.D., 592 Robin Road, Campbellsville, Kentucky 42718, representing trauma surgeons, to serve for a term expiring September 19, 2001;

Gregory A. Brown, 315 King Lane, Paintsville, Kentucky 41240, representing citizens at large, to serve for a term expiring September 19, 2003;

Diana Lee Brown, 1302 Castle Heights, Bowling Green, Kentucky 42103, representing emergency medical services educator, to serve for a term expiring September 19, 2004;

Jerry S. Fannin, Mayor, 279 Mikeal Ranch Road, Prestonsburg, Kentucky 41653, representing mayors operating a licensed Class I ground ambulance service, to serve for a term expiring September 19, 2003;

Anthony D. Stratton, County Judge/Executive, 1018 Primrose, Lawrenceburg, Kentucky 40342, representing county judge/executives operating a licensed Class I ground ambulance service, to serve for a term expiring September 19, 2001;

Michael T. Swift, 108 Marmak Drive, Glasgow, Kentucky 42141-3318, representing volunteer-staffed, licensed Class I ground ambulance service administrator, to serve for a term expiring September 19, 2001;

Mark Bailey, 7 Cedar Point, Cold Spring, Kentucky 41076-1743, representing fire service-based, licensed Class I ground ambulance service administrator, to serve for a term expiring September 19, 2004;

Connie Smith, 2628 Shady Cove Court, Bowling Green, Kentucky 42104, representing hospital administrators, to serve for a term expiring September 19, 2004;

Edward Kyle Faulkner, Post Office Box 600, Sandy Hook, Kentucky 41171, representing basic life support, licensed Class I government-operated ground ambulance service administrators, to serve for a term expiring September 19, 2003; and 

James A. Cornelison, 605 Covenant Drive, Richmond, Kentucky 40475, representing advanced life support, government-operated ambulance service administrators, to serve for a term expiring September 19, 2002.
